Steps For Vulnerable Sector Checks
STEP 1 - Complete the OHF Letter Requesting a VSC by clicking here
STEP 2 - Attach the OHF Letter Requesting a VSC to your ONLINE POLICE RECORDS CHECK
STEP 3 - Fill out OHF Screening Declaration Form by clicking here
- Member is OMHA, and Association is Frontenac
STEP 4 - Attach your OHF Screening Declaration Form and your Vulnerable Sector Check to the OHF Screening Submission Portal
- Documents uploaded must be in pdf format
- Please ensure you click on the blue OHF Screening Submission Portal link when you receive this in your email
- You will be asked to complete a declaration form again with a series of questions, and there will be a link to upload the pdf documents
