Reimbursement Form (Frontenac Minor Hockey)

Please complete the form below to request a reimbursement from FMH for expenses related to becoming certified for a FMH team. When requesting a refund please note the following: 1. All requests are subject to approval and must be submitted through this website. The request must include all of the required information requested below (any incomplete applications will not be processed). 2. FMH will only reimburse expenses related to the current season (the team official must be on a FMH approved team roster). 3. All receipts must be attached to be processed. Please allow six weeks to process the request.
